Complete and mail this Registration Form within 21 days from the Email Issue Date, determined by the postmark date on your envelope) with a check or money order in the amount of $149.90 (US funds), inclusive of a $100.00 refundable deposit, applicable towards airline taxes, handling and surcharge and a $24.95pp ($49.90) non refundable registration fee. Residents of the state of Washington are not required to submit a $100 deposit.
